A Loose Leaf (or loose certificate) is a separate document with notarial language, which is completed and attached to a notarized document. The Loose Leaf can be a Certificate of Acknowledgement, Copy Certification, or Jurat.
Notaries may use a Loose Leaf if there is no notarial language on a document or if there is not enough room to notarize a document.
