Completing PS Form 1583 includes unique identification (ID) requirements.
Primary ID
Acceptable primary forms of ID include:
- U.S. State/Territory Driver’s License
- U.S. State/Territory Non-Driver’s ID Cards
- U.S. Passports
- Foreign Passports
- U.S. Permanent Resident Cards
Refer to Section 10.3 of the DMM 608 guidelines from the USPS for more details on Primary ID.
❗ The notary’s commissioning state may be more restrictive regarding acceptable IDs than the Proof platform or the DMM. In some cases, the inverse may be true. Refer to the Not Accepted section of Acceptable Forms of ID.
Secondary ID
Acceptable secondary forms of ID include any IDs listed in the Domestic Mail Manual (DMM) Section 10.4—Acceptable Secondary Forms of Identification.
ℹ️ The purpose of the secondary ID is to verify the validity of the address you provide on the form.
