Acceptable Forms of ID for the USPS Form 1583

Completing the PS Form 1583 requires two forms of identification: a Primary Photo ID and a Secondary Address ID. Both must be current, physical documents that match the information entered on your form. Always check with the person or organization where you are submitting the document to confirm which ID documents they will accept. The PS Form 1583 includes unique identification (ID) requirements. The information below is from the DMM 608.

Steps to Prepare Your IDs

1. Ensure Information Consistency +

To ensure a successful notarization, the information on your PS Form 1583 must be consistent with the identification documents you provide.

  • Name & Photo: The name and details on your Primary ID must match the name on your form.
  • Address: The residential or business address on your Secondary ID must match the address listed on your form.

Important: The PS Form 1583 cannot be edited during the notary meeting. If you notice a discrepancy between your IDs and your form, please contact your virtual mailbox provider to update the document before you meet with a notary.
2. Provide a Government-Issued Photo ID (Primary) +

While the USPS allows some non-government IDs, the Proof platform requires your Primary ID to be government-issued to meet our security standards.

  • Acceptable Types: U.S. State Driver's License/ID Card, U.S. Passport, Foreign Passport, or U.S. Permanent Resident Card.
  • U.S.-Issued or Passport Only: Your primary ID must either be issued by the U.S. government or be a passport. Foreign driver's licenses and other non-U.S. government-issued IDs are not accepted. A foreign passport is the only non-U.S.-issued ID we can accept as a primary ID.
  • Physical Only: We cannot accept digital wallets, photocopies, or University/Tribal IDs.

Foreign IDs: If you are not a U.S. citizen or resident, a foreign passport is the only accepted primary ID. Foreign driver's licenses and other government-issued IDs from outside the U.S. cannot be accepted on our platform.
3. Provide a Valid Address ID (Secondary) +

Your secondary ID must verify the address you are using. Note: Utility bills and bank statements are not accepted for Form 1583.

  • Acceptable Types: Voter/Vehicle Registration, Home or Vehicle Insurance Policy, Mortgage/Deed of Trust, or a long-term Lease.
  • No PO Boxes: The address ID must show a physical address.

Summary Checklist

  • Confirm you have both a Primary Photo ID and a Secondary Address ID.
  • Your Primary Photo ID must be U.S.-issued or a passport — foreign driver's licenses and other non-U.S. government IDs are not accepted.
  • Verify that the information on your ID and Form 1583 is consistent.
  • Your Primary Photo ID must be a physical card — it will be captured live during the session.
  • Your Secondary Address ID can be uploaded as a file if it is a non-government document. Government-issued secondary IDs must still be captured live.
  • Both IDs must be unexpired.
