Completing PS Form 1583 includes unique ID requirements.
Primary ID
For primary forms of ID, acceptable identification includes:
- U.S. State/Territory Driver’s License
- U.S. State/Territory Non-Driver’s ID Cards
- U.S. Passports
- Foreign Passports
- U.S. Permanent Resident Cards

❗ The notary’s commissioning state may be more restrictive regarding acceptable IDs than the Proof platform or the DMM.
Secondary ID
The purpose of the secondary ID is to verify the validity of the address you provide on the form.
Secondary forms of ID, acceptable identification includes any ID types listed within the Domestic Mail Manual (DMM) Section 10.4 - Acceptable Secondary Forms of Identification.
